Return/Claim Policy
1.Scrubwear Factory by Positive Identity must give prior authorization BEFORE we will accept returned goods. The reason for return must be stated at the time of request.
2.Customers must complete a Returned Material Authorization Request Form and forward this form to Scrubwear Factory by Positive Identity for processing.
3.If the request is approved, a Return Merchandise Authorization (RMA) number and Packing Label will be emailed or faxed to the customer. This label must be attached to all related shipments to Scrubwear Factory by Positive Identity.
4.Goods authorized for return must be properly wrapped. Damaged goods will not be accepted.
5.All returns will be on a prepaid basis only, with shipping costs paid by the customer. An exception will be made if Scrubwear Factory by Positive Identity made an error when shipping the product or the original product was defective. In such cases, PRIOR authorization to return merchandise to Scrubwear Factory by Positive Identity is still required; responsibility for shipping costs will be clarified when authorizing the RMA.
6.Goods requested for return must have all tags attached and have been invoiced within the last 45 days.
7. Any items that have been decorated with embroidery or altered (i.e. hemmed) cannot not be returned.
8.Unauthorized returns will be refused and the carrier will be instructed to contact the sender for instructions.
9.All products are guaranteed to be free of defect in manufacture or decoration. Scrubwear Factory by Positive Identity will replace or repair defective item within 45 days of receipt of defective product, at no charge.
10.All Claims for short/incorrect shipments must be made within 7 days of receipt of goods.
11.Scrubwear Factory by Positive Identity is fully committed to providing quality products and service. Products with quality issues may be returned within 1 year of invoice date.
12.Special circumstances will be considered.
